Referrals

 

Who Qualifies?

To receive any services from Our Daily Bread, a client:

  • Must be a Fairfax County Resident.
  • Must have a referral by a social worker or case worker from the Fairfax County Health and Human Services or a Fairfax County Community Based Organization (CBO) or Faith Based Organization (FBO). If one does not have a social worker, please call (703) 222-0880. A social worker from Fairfax County Health and Human Services will complete an assessment over the phone and make a referral if warranted.

Our Daily Bread provides financial assistance and food assistance to all Fairfax County - except the Route 1 corridor.  We are a second tier provider in the area of UCM, Good Shepherd Housing and Family Services and LCAC.

For Food Assistance

Fill out an "Application for Assistance" and fax, mail, or email to Our Daily Bread Food Manager, Christina Garris.  Our waitlist is approximately 2 months before regular delivery.

Emergency pantry deliveries are made by volunteers within 4-5 days of referral. We deliver only to Fairfax County Residents. Any questions, call Christina Garris at (703) 273-8829.

For Financial Assistance

Our Daily Bread can provide clients with a one time grant of up to $200 which must be paid directly to the service provider. Financial grants are intended for an emergency payment towards housing, utilities, auto repair, or medical assistance.

Grants are given between the 15th and 31st of each month. 

To make a referral for financial assistance, a social worker or case worker must call one of ODB's referral intake workers. Please call between 8:30am - 4pm.  Grants are given on a first come, first serve basis. The social worker needs to be prepared to provide all the information on the Application for Assistance. Referral intake workers do not accept faxes or emails.

For Holiday Assistance

Holiday Referrals are only accepted from county and school social workers and guidance counselors. Referring agents must email a completed Holiday Assistance Request form (as an attachment) by October 15th to: forms@ODBFairfax.org

To obtain a Holiday Assitance Request form, please email appropriate contact listed in box below.

To streamline the process, avoid delivery problems and track a referrals’ status, referring agents should review the following:

  • Some groups require families to pick up assistance at a designated time and place. Other groups make deliveries. Accurate phone numbers, mailing and delivery addresses are vital to successfully receiving assistance. Completion of the referral form indicates families agree to meet the requirements of the contributing groups.
  • Referral forms for Holiday Assistance should come from your supervisor.  Complete the attached form, save it and email it to ODB. Only emailed forms will receive confirmation of receipt. The form can then be printed for your files.
  • Completed referral forms will be accepted September 1st – October 15th, 2007. Referrals will be assigned to contributing groups on a first come basis after the deadline. Several groups close their lists in October. There is no guarantee that the family will receive holiday assistance but every effort will be made to serve them.   • Forms must be in English, include the attending school, referral’s delivery address and mailing address if different.
  • As the primary liaison with this family, you may be contacted to help facilitate assistance. Please include your name, phone number, fax and email address.
  • The confirmation box indicates the information on the form is complete, has been verified and the referral desires assistance.
  • Notify ODB of any changes of address or phone number for the families you referred.  Do not send the family to ODB’s office to apply or receive assistance.
  • For the first time, you can track a referral’s status. ODB will provide an electronic file documenting the referral’s status to the following contacts on November 9th and December 7th.
